MA Athletics Required Medical information

In order to be eligible to participate in sports at Marin Academy, all students must have completed paperwork on file.
In order to be eligible to participate in sports at Marin Academy, all students must have completed paperwork on file. All forms required to participate in Marin Academy athletics can be found online at www.swol123.net.
The forms are labeled
  • Athletics Pre-Participation Physical (must be completed and signed by a doctor annually PRIOR TO the student’s participation)
The remaining forms can be completed and saved as fillable PDFs - please do not print and sign!
  • Athletics Concussion Acknowledgement Form
  • Athletics Sudden Cardiac Arrest Acknowledgement Form
  • Athletics Permission to treat + Privacy practices
  • Athletics ImPACT Informed Consent upd 5-14 MA
We will accept the form as complete if the student and parent’s names are typed into the form’s fields. You will then electronically sign the form when you click “Save & Submit"
Students cannot participate in practices/tryouts until all of these forms are complete. The completed forms are valid for one year, and must be re-signed each year

Once you’ve completed these forms and they’ve been approved by Aaron Gill, ATC, you will see “Cleared: Yes” on the SportsWare log-in screen.

he Athletic department is asking all parents and/or students interested in playing a Fall Sport to complete this GoogleForm  This helps the department ensure names of students interested in or planning to play a sport have the necessary forms completed prior to the start of the Fall season August 13th.
